Education and conferences

The training activity on breast reconstruction carried on by Prof. Santanelli di Pompeo with DIEP flap, Latissimus Dorsi flap, lipofilling and others procedures, is part of a custom made scientific program to raise the number of surgeons able to perform procedures in the field of microvascular surgery and autologous tissue transfer.

Special attention is reserved to the education of young surgeons, with the intention to create a school of surgery aiming at popularizing the DIEP flap procedure in Italy, and to increase the number of plastic surgeons specialized in such technique, in order to make it easily available to those women that have to cope with the difficult experience of beeing addressed to a radical mastectomy.

Chair of Plastic Surgery, “Sapienza” University of Rome, School of Medicine and Psicology.

University Program in Plastic Reconstructive and Aesthetic Surgery.

At the “Sapienza” University of Rome, School of Medicine and Psychology.

Director: Prof. Fabio Santanelli di Pompeo.

It started on 2007 and is the first Master, in Italy, dedicated to selective education on breast reconstruction techniques, with special attention to DIEP, Latissimus Dorsi flaps and fat grafting. The Master offers a practical and detailed education on these procedures, through direct participation to surgery along 12 months course. The Master is repeated annualy and addressed to any post-graduated students in Medicine and/or full board in Plastic Surgery, with a maximum number of 15 candidates per year, usually coming from all over Italy and other European countries.

The Fellowship uses a professional training program to provide the specialist skills to face reconstructive procedures of various anatomical districts of the body. It includes a professional training to physicians in order to acquire specialist skills in basic microsurgical techniques through an animal model. The Fellowship additionally provides frontal teaching lectures and their resulting application in the clinical and surgical field with patient preparation, the surgical procedure and follow-up, mainly on the breast but also head and neck, and limbs.

Is an online educational project conceived by prof. Fabio Santanelli and managed by himself and his team. It is a web-site www.echirurgiaplastica.it meant for the education and teaching of the most common pathologies in Plastic Surgery and their correction, to Medical students and Plastic Surgery trainees.

The possibility to know pecisesly the breast volume on which to operate is of outmost importance for surgeons, either in aesthetic either in reconstructive plastic surgery. Untill yesterday several methods have been described, some very complicate to performe and some other unreliable for the results obtained.

Prof. Fabio Santanell and Dott. Benedetto Longo have carried out a scientific study that allowed to realize a simple and statistically reliable method to measure breast volume. Ten different anthropometric measures were taken from 88 women, before undergoing to mastectomy for breast cancer; results have been later correlated to breasts volumes measured from surgical specimens, and only 3 (sternal notch-to-nipple distance, inframammary fold-to-fold projection distance and inframammary fold-to-nipple distance) out of 10 were found statistically significant. An algorithm was then developed, BREAST-V®, based on the simple determination of the 3 measures and able to precisely measure the volume of the studied breast.

To simplifye its use the Diepflap.it Medical Software has produced and delivered to Apple an “App” for iOS devices (iPod, iPhone, iPad) available on App Store, able to quickly performe the calculation.

Today thank to the BREAST-V® it is possible a fast, non invasive and statistically reliable breast volume determination. Such innovative method is of out most importance in all breast surgery, either aesthetic procedures as breast augmentation or correction of asymmetries, either reconstructive breast procedures after cancer.

Sentinel Lymph Node Biopsy is today considere as the gold standard in axillary staging of breast cancer patients. Although a recent American paper highlights that in case of Sentinel Lymph Node metastasis a Complete Axillary Lymph Nodes Dissection doesn’t impact on survival, this procedure is actually indicated, and only in 35-50% of these patients additional nodal metastases are shown.

To assess the risk of Metastases to the residual Axillary Lymph Nodes several statistic methods were described, starting from the first tool (Memorial Sloan-Kettering Cancer Center Breast Service, 2003). Thus, aim of these tools is to calculate the probability of additional axillary disease in Sentinel Lymph Node positive patients, and the final goal is to avoid unnecessary Complete Axillary Lymph Nodes Dissection in low risk breast cancer patients.

After the validation of the most used published tools, an original method was developed based on the case mix from Sant’Andrea Hospital and other Breast Units, that takes in consideration the following variables to calculate the risk: primary tumor size, histological type, grading, Sentinel Lymph Node metastases size, and positive Sentinel Lymph Node/total Sentinel Lymph Node ratio.

To allow its diffusion and validation the statistical model has been transformed into an “App” for IOS platform, downloadable for free by the Apple Store, designed for mobile devices as iPhone, iPad and iPod, with the goal to provide an helpful and easy decisional tool, with a clinic feasibility and not only in a research environment.

The application “Thromboembolism Risk” is based on the official Thromboembolism Risk Assessment Protocol adopted by the Sant’Andrea Hospital of Rome after a multidisciplinary consensus for the prevention of DVT/PE in surgical patients.

The protocol represents a modification of the original Caprini Risk Assessment Model.

The application for iPhone and iPad aims at making easier the collection and elaboration of the various risk factors and offering thorough guidelines for the peri-operative treatment of surgical patients.
